2026 Missions Trip Application

Thank you for your interest in Mountain West's Short Term Missions Trips! Please complete the following application, read the Mission Team Covenant, indicate your agreement with the covenant, submit your application, and pay your deposit as soon as possible.

Once your application has been considered, you will be contacted by the trip leader for an interview and discernment conversation, and then notified of your placement on the team. If you have any questions about the application or details of the trip, please contact the appropriate trip leader listed below.

Participation Requirements

To participate in a Mountain West Missions Trip, the following is required:
1) You must have actively participated a Mountain West Hope Team within the past 12 months.
2) You must provide a recommendation/reference from a Mountain West Staff Member, Hope Team leader, or Small Group leader.
3) You must participate in our Mission Trip Training sessions (these will take place early 2026).
4) You must have completed the trip payment in full to Mountain West Church (through fund-raising or personal expense).
5) You must have a valid passport with an expiration date 6 months past the return date of trip.

I understand that the training sessions are of vital importance to the success of this team and I commit to faithfully attend meetings leading up to the trip. If an unavoidable conflict occurs, I agree to notify group leaders ahead of time and seek out information I miss.

I agree to participate in regular worship and Bible study knowing that my spiritual well-being and growth is a primary purpose in team participation.

I will refrain from any activity or behavior that might be unbecoming of a Christ follower or a stumbling block to any team member, local persons, ministry partner, or Mountain West Church.

I will be sensitive to the local culture in my words, actions, and the way I dress. I will develop an attitude of service and flexibility.

I will not hold Mountain West Church and any of their departments, segments, officers, agents, and employees responsible for any accident, injury or illness resulting from my visit to the field. Nor will I hold the aforementioned parties responsible for the loss of or damage to personal property while on this assignment and indemnify and hold harmless Mountain West Church from and against any and all liability occasioned by my service.

Financial Commitment *

Required Trip Deposits Due By Application Deadline: Guatemala $500.00 & South Africa $500.00

I understand the deposit will be held until acceptance on the Mission Team is confirmed. At that point, the deposit is non-refundable and will be counted towards the team member's total cost of the trip. Should applicant not be placed on the team, the deposit will be returned.

I understand that I am financially responsible for the full trip cost and agree to raise funds as prescribed in the support-raising manual or self-fund as necessary to meet this goal.

I understand that additional fees not covered by the trip cost may include but are not limited to immunizations, passports, and personal items needed for travel are also my responsibility.

I affirm that Mountain West Church/Servant's Hearts Ministries/Hope South Africa has discretion and control over the use of all donated funds, and reserves the right to reallocate funds designated for my trip in the event that I am fully-funded and others on the team have a shortage.

I understand and will explain donors that should I have a surplus of funds for my ministry costs, they will not be refunded but instead reallocated to other team members in need.

I agree to meet fundraising deadlines as determined by the missions department, and I understand that failure to meet the deadlines may result in being unable to go on the trip.

In the event that I am unable to participate in the trip I understand that all funds raised are charitable gifts and non-refundable.

Cancellation or dropping out after the purchase of non-refundable advance deposits (housing, transportation, excursions, etc.) will require full payment even if you are unable to participate in the trip.

Acknowledgement of Background Check

Depending on the trip, and with prior notification to you, Mountain West Church may obtain information about you from a consumer-reporting agency for volunteer and/or employment purposes. The report may contain information bearing on your character, general reputation, personal characteristics, or mode of living, including but not limited to criminal and driving records, education, prior employment verification, and workers' compensation claims. Only in situations where the employee/volunteer will be involved with financial information will credit checks be included as part of the background check process. The report may be obtained from public and/or private sources (or in the case of an investigative consumer report, through personal interviews with your neighbors, friends, and associates). You have the right to request disclosure of the nature and scope of any investigative consumer report.

I authorize without reservation, any party or agency contacted by Protect My Ministry, INC. to furnish the aforementioned information.

I understand that I have the right to make a request to Protect My Ministry, Inc., upon proper identification, to request the nature and substance of all information in its files on me at the time of my request to include the sources of information. I hereby authorize Protect My Ministry, Inc. to request and obtain the above-referenced consumer reports at any time during the application process or during my term as a volunteer or my term of employment with Mountain West Church or as often as Mountain West Church deems necessary.
